The Phish Book on page 2 or 3 has a picture of Trey with a bloodied nose, with a caption that reads 8/23/92. I was at that show and I heard a rumor afterwards that Trey was punched by Carlos Santana or a member of his band. I can believe that rumor because it seemed to me that Trey had upstaged that night. Anybody ever seen the picture or heard the rumor?

No, Trey was upstaging Carlos Santana, going off, going Trey. It was a weird night.99% of the mostly hispanic crowd were there to see santana, and then the 30 of us who were there to see Phish. The crowd was booing during the Phish opening set.
